Here are the following requirements an applicant must submit:

• Complete, up-to-date resume

• A photocopy of California ID or Driver’s license

• A photocopy of Social Security number or Tax Identification ID

• One current photo (1 x 1, 2 x 2, or passport)

• Appropriate recommendation letters, if any

• Certificates, if any, such as Basic Life Support
